- Driver's license number for each driver
- Year, make, model and VIN for each vehicle

- Any SR-22 or FR-44 requirement

- Flood zone, if you know it
- An elevation certificate, if you have one
- Your closing date or Citizens renewal date
Health plan enrollment

Medicare review
